Image release and media consent form

This form is for volunteers, contractors, trustees and other people associated with the Adult Literacy Trust (ALT) agreeing to participate in the production of ALT photography, video or audio work.

ALT is committed to processing personal information in accordance with all applicable data protection laws and will ensure that the rights and interests of data subjects are weighted appropriately.

Declaration

Please read and confirm the following statements:

I confirm that I am aged 18 or over.*

I hereby consent to the capture of my image in photographs/footage by ALT. I understand that my participation is voluntary, and that I may withdraw my consent to be photographed, filmed and/or recorded at any time.*

I hereby grant to ALT the right to process, use and reproduce my image online and/or via other digital/analogue methods for educational purposes, and that I consent to the sharing of my image with other volunteers, contractors and trustees of ALT.*

N.B. This refers to sharing recordings of trainings with other students who were not able to attend the session.
